There are times when a traditional root canal treatment cannot save your tooth, and surgery may be required. For example, there may be a small crack in the tooth that was not seen during a nonsurgical procedure or on an x-ray. However, surgery will let us examine a tooth’s complete root structure, locate the issue, and remedy it. One of the most common surgical procedures an endodontist performs is an apicoectomy, also known as a root-end resection.

An Apicoectomy Explained

During an apicoectomy procedure, the gum tissue is opened near the tooth so the endodontist can see the underlying bone in order to remove any infected or inflamed tissue. At that time, the very end of the tooth’s root, the apex, is removed. We then place a small filling in the root so that the end of the canal is sealed. Sutures are placed to help with the healing process. Over time, your bone heals around the tooth’s root. Patients will be administered local anesthetics so that they are comfortable during the treatment. Most patients are able to drive after the treatment, but if you have concerns, please arrange for a friend or family member to take you home. We can recommend pain medication to take while the gum tissue is healing. Recovery time is usually short, and you should be able to resume your regular daily activities within a day or two.
